A rapidly growing palliative care organization is seeking an experienced Advanced Practice Provider (NP or PA) Team Lead to provide both clinical leadership and direct patient care across New Jersey. The APP Team Lead will serve as a key clinical resource while supporting operational excellence, provider mentorship, and interdisciplinary collaboration. This role offers a blend of leadership responsibilities and hands-on patient care in a patient-centered, mission-driven environment.

Position Overview
Clinical Leadership
• Provide day-to-day leadership, guidance, and support to Nurse Practitioners and Physician Assistants
• Promote evidence-based palliative care practices and consistent clinical standards across the APP team
• Serve as a clinical resource for complex patient cases, symptom management, care transitions, and goals-of-care discussions
• Participate in case reviews, interdisciplinary rounds, and quality improvement initiatives
• Foster collaboration among physicians, nurses, social workers, behavioral health professionals, and care coordinators
Direct Patient Care
• Conduct comprehensive palliative care assessments and follow-up visits
• Manage patients with advanced illnesses, chronic disease burden, and complex symptom needs
• Facilitate advance care planning and serious illness conversations with patients and families
• Coordinate care with primary care providers, specialists, hospitals, hospice agencies, and community resources
• Ensure timely and accurate clinical documentation
Team Development & Operations
• Mentor, coach, and support APP team members in clinical and professional development
• Assist with onboarding, orientation, and training of new providers
• Conduct team meetings, performance check-ins, and peer support sessions
• Support scheduling coordination, productivity management, and workload balancing
• Help develop workflows, protocols, and best practices to improve care delivery
Quality & Program Growth
• Monitor clinical quality metrics, documentation compliance, and patient outcomes
• Support initiatives focused on reducing avoidable hospitalizations and improving patient satisfaction
• Collaborate with leadership on strategic planning, service expansion, and operational efficiency
• Contribute to organizational growth and program development efforts
Qualifications
Required
• Current New Jersey Nurse Practitioner or Physician Assistant license
• National board certification as an NP or PA
• Active DEA license and CDS registration
• Minimum of 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or complex chronic disease management
• Strong leadership, communication, and interpersonal skills
• Ability to work independently and effectively within interdisciplinary care teams
Preferred
• Prior leadership, mentoring, or supervisory experience
• Certification in Hospice and Palliative Care (ACHPN or equivalent)
• Experience with home-based care, value-based care, or population health programs
• Familiarity with telehealth platforms and EMR systems
• Bilingual language skills are a plus
