Nursing Home Physician - NYC Boroughs

Location: New York, New York 10036 (Serving nursing homes across NYC Boroughs)

Position: Internal Medicine (1 opening)

Provide specialized geriatric care to nursing home residents throughout NYC boroughs (Manhattan, Bronx, Brooklyn, Queens, Staten Island) from a Manhattan base.

Key Responsibilities:

  • Manage chronic conditions, med reconciliation, and acute issues in SNFs
  • Conduct rounds, order diagnostics, treat infections/wounds, and oversee care plans
  • Coordinate with facility staff, specialists, and families for transitions/hospice
  • Complete documentation, regulatory compliance, and quality reporting

Qualifications:

  • Board-certified or board-eligible in Internal Medicine or Family Medicine
  • Active NY medical license and DEA
  • Geriatrics/nursing home experience preferred; reliable transportation required
