Family Nurse Practitioner

Position Summary

The organization is seeking a Family Nurse Practitioner to join its expanding outpatient primary care team in Naugatuck, Connecticut.

This full-time, permanent role is outpatient-focused and offers a supportive clinical structure with experienced physicians, advanced practice providers, and ancillary staff. Providers benefit from a patient-centered culture guided by academic best practices.

Essential Responsibilities

• Provide comprehensive primary care services to patients across the lifespan

• Diagnose and manage acute and chronic medical conditions

• Conduct preventive care visits, annual physicals, and wellness screenings

• Prescribe medications and develop evidence-based treatment plans

• Collaborate with physicians and multidisciplinary care teams

• Maintain accurate and timely EMR documentation

• Educate patients on health maintenance and disease prevention

Qualifications

• Education: Master’s or Doctoral degree from an accredited FNP program

• Certification: National certification as a Family Nurse Practitioner

• Licensure: Eligible for Connecticut APRN licensure

• Experience: Prior primary care experience preferred; new graduates welcome
